Use this form to Turn-In existing microcomputer or telephony equipment (after submitting the form, please allow 2 to 3 weeks for equipment to be picked up).

Process to Turn-In Microcomputer or Telephony Equipment:

Fill out the form below and print a copy of the confirmation page after pressing the submit button.
Attach a copy of the confirmation page to the equipment to be picked up. If a copy is not attached, it will not be picked up.
