Turn-in of Microcomputer Equipment Instructions

Purpose: Define the process to TURN-IN microcomputer (computer, monitor, keyboard, and associated devices such as a mouse, printer, scanner, and external modem) or telephony equipment to another room, building, or campus.

General: This document provides the necessary guidelines for the requester to initiate the turn-in of microcomputer or telephony equipment.  These guidelines must be followed to ensure coordination with the necessary offices and that all required documents are completed.  This form must be used when microcomputer or telephony equipment needs to be turned in for redistribution or disposal.

Arranging for Microcomputer or Telephony Equipment to be turned in:  ITS is responsible for picking up or preparing desktop computers and any associated components such as keyboards, mice, scanners, and printers for turn in.  In order to provide adequate service, sufficient notice is required to ensure the microcomputer equipment is picked up in an orderly and accountable manner.  (After submitting the request, please allow 2 weeks for pickup of microcomputer equipment from the Fort Omaha campus and 3 weeks for pickups from the other campuses or locations.

NOTE:  Microcomputer or Telephony equipment will not be picked up or prepared for turn in unless a copy of the “Turn-in of Microcomputer Equipment” form is completed and attached to the microcomputer equipment.

 

Cost Center Manager Responsibilities:

  1. Ensure the microcomputer or telephony equipment to be picked up belongs to your cost center.

  2. Determine whether the microcomputer or telephony equipment is to be turned in.

  3. Ensure the request has been filled out accordingly.

Requester Responsibilities:

  1. Prepare and submit the Turn-in of Microcomputer or Telephony Equipment form.

  2. Make a copy of the confirmation page for your records.

  3. Make another copy and attach it to the equipment to be picked up.

Information Technology Services (ITS) Responsibilities: 

  1. Pick up or prepare the microcomputer or telephony equipment for pickup by Central Stores.

  2. Prepare the equipment for pickup by Central Stores unless technician plans to deliver.

  3. Notify Central Stores when the  equipment is ready for pick up unless planning to deliver.

  4. Update the inventory database as required.
