Request for Microcomputer Equipment Instructions

Purpose: To request microcomputer equipment (i.e., computer or devices attached to the computer such as a monitor, printer, scanner, or digital camera) or a FAX machine to meet a legitimate need of the requester.

 

General:  These guidelines must be followed to ensure that the coordination with necessary offices and the required documents are completed.

 

The requester is responsible for completing and submitting this form.  (After submitting the form, please allow one to three weeks for some informative feedback on the processing of your request. The time it takes for the PC Matching Group to develop a recommendation depends on a number of factors and can take several weeks).

 

Requester:

  1. Determine what microcomputer equipment is required

  2. Check with your Cost Center Manager to determine if the microcomputer equipment you require has been budgeted for and obtain the budget line item number

  3. Complete the request form accordingly and submit

  4. Make a copy of the confirmation page for your records

Cost Center Manager:

  1. Determine if you concur with the requester's need for microcomputer equipment

  2. Let the requester know if the microcomputer equipment has been budgeted for and what is the budget line item number

  3. Give the requester your approval  to submit the request form to the PC Matching Group

PC Matching Group (PCMG) Responsibilities:

  1. Review the request

  2. Gather any other needed information

  3. Determine if requested microcomputer equipment is available for redistribution or if it must be purchased

  4. Use all available data to determine if request can be fulfilled in its entirety or what portion can be fulfilled

  5. Communicate the PCMG's determination back to the requester along with an estimate of when it should be fulfilled

  6. Initiate Help Desk ticket and assign to the appropriate technician to ensure the approved request is fulfilled

Executive Team Responsibilities: 

  1. Review any PCMG decisions a requester wishes to appeal

  2. Communicate decisions on appealed requests to the PCMG

Technician Responsibilities: 

  1. Install, setup, and/or configure the microcomputer equipment according to the  information detailed in the ticket from the Help Desk
